Update Your Vaccination Records, Pharmacy, Email Address, Health Card Number, or Demographics


If you have received a COVID-19 vaccination, or any other vaccination such as flu shot or shingles vaccine, you can now update your medical record securely! 

You can also let your doctor know of any address or telephone number changes, update your health card number or version code.

You can also update your email address or give email consent to your doctor.

At Arbour Family Medical Centre, our patients come first. The following office policies and services have been developed for the convenience of our patient community, to keep our office running smoothly, and to provide an exceptional standard of care.

Medical advice or test results will not be given over the phone. Should you have an abnormal test result, we will contact you.

Patients should make every effort to see their physician prior to calling After Hours Service for after-hour care.

Ontario Health Card Information

If you have lost your health card or it has expired, you must contact a Ministry of Health office immediately.

Please call MOH to check what identification you will need to provide to them. Once you receive your new health card please contact our office ASAP.

At this time, we are not accepting new patients.

To make your first appointment with us go as smoothly as possible, please fill out our New Patient Form, available for download below, in advance.

To allow someone else to book appointments and receive information from your medical record, please fill out our Consent to Disclose Personal Health Information form, available for download below.

To allow us to communicate with you through email, please fill out our Consent and Release to Receive Email form, available for download below.

Appointments & Scheduling

Patients are seen by appointment only. Patients will be billed for missed appointments without 24 hours' notice of cancellation.

Due to increased demand for "same day" appointments for urgent care, the wait time for routine appointments have increased. Please allow approximately 2 - 4 weeks for routine appointments for issues such as renewal of prescriptions and blood pressure checks. Please allow 2 - 6 months for a routine physical. Wait times depend on each individual doctor.

You must provide a valid Health Card at the time of your visit. Please bring a list of medications to each appointment.

ARBOUR/PHOENIX FHO After Hours Service

The After-Hours Medical Service is for urgent care, and should not be considered a convenient substitute for routine care provided by your doctor. Your family physician is best suited to manage your health care and is generally available to see you during regular office hours. If you require any medical attention, please contact your family physician first. 

If you are suffering from chest pain or shortness of breath, please go to the closest Emergency Room, or call 9-1-1.

If you need after-hours care, please call our After Hours Number at 1-888-308-5228

When calling our after hours number, you will be connected to the Phoenix FHO doctor on call or TeleHealth Advisory Services. Should they feel you need to be seen, they will determine the urgency of your concerns and advise you of the time and place for the provision of care.

Please note that during our After Hours Service, we do NOT manage or provide the following:

  • Immunizations/Travel medicine
  • Allergy Injections
  • Ear Syringe
  • Routine Physical/Gynecological examinations
  • COVID-19 Testing
  • Prenatal Care
  • Chronic disease/pain management
  • HIV testing or treatment
  • Sexually transmitted disease screening
  • Contraception management
  • Mental health or psychiatric issues
  • Seizure disorders
  • Filling out forms (e.g., WSIB, sick notes)
  • Prescription renewals
