If You Have A Copy Of Your Medical Records

If you have digital copies of your medical records (such as a PDF, a CD or a USB) you can email records to us as at afmciadmin@afmci.com

Alternatively, if you have a physical copy - or are unable to email your records - they can be dropped off at Arbour Family Medical during our regular business hours

If You Were Seen By A Specialist

If you were referred to a specialist by a Arbour Family Medical Physician, it is very likely that they have forwarded us a copy of your report/consult/results via fax.

If You Were Seen In A Walk-in Clinic , in The ER , or Out Of Province

The healthcare provider you saw may have asked for the information of your primary care physician so that they can send us the reports directly. In which case, there is no need for you to send additional copies.

The best way to forward your physician any reports is to ask the facility you visited to send it to us directly. Your physician’s fax number can be found here.

Alternatively, if you have a copy and you would like to forward it to us, you can send them via email to us as a PDF (we have fairly strict I.T. limitations to opening other types of files).
