Update Your Vaccination Records, Pharmacy, Email Address, Health Card Number, or Demographics
If you have received a COVID-19 vaccination, or any other vaccination such as flu shot or shingles vaccine, you can now update your medical record securely!
You can also let your doctor know of any address or telephone number changes, update your health card number or version code.
You can also update your email address or give email consent to your doctor.

Appointments & Scheduling
Patients are seen by appointment only. Patients will be billed for missed appointments without 24 hours' notice of cancellation.
Due to increased demand for "same day" appointments for urgent care, the wait time for routine appointments have increased. Please allow approximately 2 - 4 weeks for routine appointments. Wait times and availability depend on each individual doctor.
You must provide a valid Health Card at the time of your visit. Please bring a list of medications to each appointment.
ARBOUR/PHOENIX FHO After Hours Service
The After-Hours Medical Service is for urgent care, and should not be considered a convenient substitute for routine care provided by your doctor. Your family physician is best suited to manage your health care and is generally available to see you during regular office hours. If you require any medical attention, please contact your family physician first.
If you are suffering from chest pain or shortness of breath, please go to the closest Emergency Room, or call 9-1-1.
If you need after-hours care, please call our After Hours Number at 1-888-308-5228
When calling our after hours number, you will be connected to the Phoenix FHO doctor on call or TeleHealth Advisory Services. Should they feel you need to be seen, they will determine the urgency of your concerns and advise you of the time and place for the provision of care.
